Ejaculation by Ray Sahelian, M.D. - Female and Male Ejaculation Problem information

Male ejaculation has two distinct phases, emission and expulsion. Orgasm, a feature perhaps unique in humans, is a cerebral process that occurs, in normal conditions, along with expulsion of semen. Normal ejaculation is a highly coordinated physiological process with emission and expulsion phases being under the control of autonomic and somatic nervous systems respectively. The central command of ejaculation is located at the thoracolumbar and lumbosacral levels of the spinal cord and is activated by stimuli from genital, mainly penile, origin although cerebral descending pathways exert both inhibitory and excitatory regulatory roles. Cerebral structures specifically activated during ejaculation form a tightly interconnected network comprising hypothalamic, diencephalic and pontine areas.

Ejaculation neurotransmitters that delay ejaculation
Several neurotransmitters contribute to the male ejaculation process. Amongst them, serotonin has received strong experimental evidences indicating its inhibitory role in the central control of ejaculation. In particular, 5-HT1A cerebral autoreceptors but also spinal 5-HT1B and, in a lesser extent, 5-HT2C receptors have been shown to mediate the effects of 5-HT on ejaculation. Selective serotonin reuptake inhibitors (SSRIs) increase the amount of central serotonin and delay ejaculation in humans. So does 5-HTP, a serotonin precursor.

Cause of premature ejaculation
About 20 to 30% of men have premature ejaculation. The cause of premature ejaculation remains to be clarified, but current evidence reflects a shift from psychological reason to more neurobiological causes. Hypersensitivity of the penis or head of penis does not appear to be a cause of premature ejaculation. There are those who have suffered with premature ejaculation from the onset of their sexual life while others developed this problem later on. The latter case is called secondary premature ejaculator.

Premature ejaculation cure
Premature ejaculation is a common sexual problem and many men search for a natural solution to prevent premature ejaculation. P
reventing premature ejaculation with 5-HTP, a supplement that converts into serotonin, is one option. Kava capsules taken an hour or two before sex may also be tried. Another option is to use an anesthetic cream on the head of the penis to reduce sensation.

Premature ejaculation treatment
Five main treatment approaches have been used in clinical trials to stop premature ejaculation: behavioral therapy, antidepressants, phosphodiesterase-5 (PDE5) inhibitors, topical anaesthetic agents and alpha-blockers. Most antidepressants appear to delay ejaculation to some extent at all doses. Anesthetic creams appear to be as successful in slowing ejaculation as antidepressants without systemic side-effects, although some patients do experience erectile problems or unpleasant local symptoms. Anecdotally, behavioral therapy for pre mature ejaculation is effective and appears to have long-lasting efficacy. You can delay ejaculation by not stimulating the penis too much or taking 5-htp, kava, or SSRIs.

Retrograde ejaculation
Retrograde ejaculation occurs when semen flows to the bladder instead of emerging through the penis during orgasm. Retrograde ejaculation isn't harmful. But it can impair fertility. During normal ejaculation, the opening of the bladder (bladder neck) closes to prevent semen from entering as it passes through the urethra. In retrograde ejaculation, the bladder opening doesn't close properly. As a result, sperm is allowed to enter the bladder instead of being ejected out the tip of the penis. Causes of retrograde ejaculation include prostate surgery, bladder surgery, diabetes, multiple sclerosis, and certain medications such as psychiatric drugs and medications used to treat high blood pressure and prostate enlargement.

Pre ejaculation
A woman can get pregnant even when ejaculation doesn't occur within the vagina. This is because pre-ejaculation fluid may contain some sperm. Withdrawal of the penis from the vagina before ejaculation is one of the oldest methods of birth control but pregnancy can occur in a small percentage of cases due to the pre ejaculation fluid.

Male ejaculation problem
The time from initiating sexual activity to ejaculation varies from one individual to another. This time period is called the ejaculatory latency. What may be an ejaculation problem for one man may be acceptable to another. Typically, ejaculation disorders fall into two categories. These are: delayed ejaculation and early ejaculation. In the vast majority of cases, the most effective therapeutic approach for ejaculatory dysfunction is a combination of biologic and psychologic therapy. From an emotional standpoint, it is important to understand the history and background of the individual. Issues such as depression, anxiety, past sexual experiences, psychological trauma and relationship history are important considerations that need to be discussed early in the evaluation.
Types of male ejaculation problems include the following: premature ejaculation, inhibited ejaculation, anejaculation, retrograde ejaculation and anorgasmia.

Ejaculation problem due to drugs
Using Flomax (tamsulosin) every other day, rather than every day, appears to reduce the ejaculation problems that can occur with the drug. Given that the drug's effectiveness is not compromised with intermittent therapy, this dosing may become the standard for all men. Drugs such as Flomax are used to treat urinary symptoms related to an enlarged prostate -- a condition known as benign prostatic hyperplasia (BPH). Typical BPH symptoms include difficulty in starting to urinate, having a weak urinary flow, and the need to urinate urgently or frequently. However, Flomax can cause ejaculation abnormalities.

Vaginal ejaculation
Vaginal ejaculation is fluid that comes out of the urethra (pee tube) which originates not from the bladder but from the Paraurethral and Skenes glands (located under the g-spot area).

Female Ejaculation - Woman Ejaculation
In a woman, the Grafenberg zone is a sexually arousable zone in the front part of the vagina, stimulation of which can lead to the discharge of liquid from the urethra, a phenomenon which is described as female ejaculation. Since the 1980s the so-called Grafenberg zone, popularly termed "G-spot", and female ejaculation have been controversially discussed medically as well as in popular magazines.

Male Ejaculation control
Lack of male ejaculation control is one of the top concerns of many men under the age of 50. Concerns range from the man who ejaculates within seconds, at the first touch, just prior to penetration, or immediately after penetration. In these male ejaculation control cases, 5-hydroxytryptophan may be helpful.

Increase ejaculation volume
One can increase ejaculation volume by eating more cold water fish, taking fish oil capsules, or using certain aphrodisiac herbs such as tongkat ali, horny goat weed, or maca.

Prolong ejaculation
You can prolong ejaculation by not stimulating the penis or vagina beyond a certain point.

Painful ejaculation
Painful ejaculation may occur due to the use of certain prescription drugs such as antidepressants -- the tricyclic antidepressants (TCAs, e.g. clomipramine, imipramine, desipramine, protriptyline, amoxapine), the selective serotonin reuptake inhibitors (SSRIs, e.g. fluoxetine), venlafaxine and the MAOIs. Apart from lowering the dose and changing the antidepressant, no strategies are available to treat this side effect.
Prostatitis or inflammation of the prostate can also cause painful ejaculation in a man. Ejaculation pain may also occur with some infections that are sexually transmitted.

Ejaculation questions
Q. Is female ejaculation orgasm possible?
   A. A woman can ejaculate, but not all women are able to ejaculate during orgasm. Feminine ejaculation is not overly common.

Q. How can I increase my penis ejaculation? I am looking for a huge ejaculation. Are herbs such as tongkat ali and yohimbe helpful in penis ejaculation or sexual enhancement?
   A. If you mean how you can increase ejaculation fluid, fish oils and eating fish such as salmon or halibut, or shellfish can be helpful. There have not been studies yet with tongkat ali to know whether it increases penis ejaculation volume, but some people say their ejaculation is more intense. In a dog study, yohimbe administration led to a big ejaculation. The big ejaculation lasted several hours after the administration of the yohimbe bark extract.

Q. My ejaculation is delayed. Is retarded ejaculation a common problem?
   A. Delayed ejaculation is not as common as premature ejaculation although delayed ejaculation becomes more common as we age. I don't see the term retarded ejaculation used much.

Q. What's the difference between ejaculation and orgasm? Also, is a first ejaculation more intense that subsequent ones? Are there herbs that increase ejaculation distance?
   A. Ejaculation is the discharge of fluid while orgasm is the climax of the pleasurable physical and psychological response to prolonged sexual stimulation. An orgasm is often accompanied by a large ejaculation volume, blushing or spasm. An orgasm could occur with little ejaculation volume or without ejaculation, but most often there is plenty of fluid that is ejaculated. First ejaculation can be intense, but others in the future can be just or more intense than a first ejaculation. It is possible that Passion Rx or tongkat ali use could increase ejaculation distance.

Q. I have a weak ejaculation. Are there herbs that can be helpful? Also for better libido.
   A. Certain herbs could help with weak ejaculation, some that come to mind include yohimbe and tongkat ali.
